ALBEMARLE HEALTH HEART CLINIC NOW OPEN
The Albemarle Health Heart Failure Clinic is an outpatient service that provides comprehensive management of clients with heart failure of any etiology. The program is staffed by a Nurse Practitioner and a Nurse Educator, experienced in the management and education of clients living with heart failure. The clinic staff follows the American Heart Association and American College of Cardiology Heart Failure Guidelines, and works closely with a primary care provider to formulate the optimal, individualized treatment plan for controlling your heart failure.
The goal of the Albemarle Health Heart Failure Clinic is to improve the patient’s clinical outcomes and quality of life by providing comprehensive education and counseling, optimizing available medical therapies, and maintaining close contact and follow-up of patients with heart failure in an outpatient setting.
Services available in the Heart Failure Clinic include:
- Disease management, consisting of education about both the heart failure disease process and the medications used to manage heart failure.
- Medication initiation, titration, monitoring and follow-up.
- Functional and Quality of Life Assessments.
- Nutritional instruction on dietary modifications necessary to manage heart failure.
